Admission Procedures
As our hospital is committed to making every patient’s stay as comfortable and secure as possible, the reception counter No. 8 “Patient Support Center (総合サポートセンター / Sogo Support Center)” on the 1st floor is open to consultation for inquiries such as admission procedures and recovering. Please feel free to contact our staff.
- We will explain your treatment and/or operations on the day of admission. We kindly ask you and your family come together.
- We will check medication history for a week prior to admission.
- Please remove and refrain from using nail/toenail polish, artificial nails (gel nails), false eyelashes (eyelash extensions) or any earrings during the hospitalization.
- Admission procedures will be completed at reception counter No. 8 “Patient Support Center” on the 1st floor.
- Fill out and stamp (with your personal seal) the “Admission Application Form (入院申込書 / Nyuin Moushikomisho)” (issued before hospitalization) and submit it to the staff of Patient Support Center on the day of admission. If you were hospitalized at a different medical institution, a “Discharge Certificate (退院証明書 /Taiin Shoumeishosho)” is also required.
- If you have a fever or suspected infectious disease before hospitalization, your admission day may be rescheduled. In that case, contact the Patient Support Center by phone.
